ITA Forms

The following ITA forms are attached below. Click on each form to open and download.

1) Training Exploration Form: All clients must explore at least 2 training programs and complete the Training Exploration form for each program, prior to applying for and ITA. A list of ITA training providers & programs is available on Illinois workNet at:

2) Financial Statement: Complete this financial statement and upload it to the BHES site with the ITA Application.

3) Attendance Sheet: Give the attendance sheet to the client to give to the Training Provider for use in the billing process.
