Requesting Staff Access
Supervisors of agency or Partnership staff who need access to Career Connect or other data systems for administering WIOA and non-WIOA programs must complete, sign and submit the Database Access Request Form (attached below) along with the additional agreements and information required for each system that the staff person needs to access.
Each form should be submitted as a separate attachment because they need to be routed to different staff members.
The required forms are listed in the Database Access Request Form and are attached below.
Submit completed forms to the Career Connect Help Desk (helpdesk@chicookworks.org) or your designated Partnership contact.
When Staff Leave
When staff leave your agency, please make sure to notify the Help Desk right away so we can inactivate their accounts. Please include the following in the Help Desk request:
- staff person's name
- last day of work
- what data system(s) they had access to
If the staff person had an Illinois workNet account, please also email IwN directly at info@illinoisworknet.com.
**UPDATED FORM** (9/25/25): The Shared Data Agreement has been updated to form 18-SDA-62 linked below. Please note that electronic signatures are not being accepted at this time. You will need to type into the fields, print, sign, scan, and submit the form.
