Please use these updated Case File Cover Sheets to organize your files.  The updates include changes to the following:

There is now a place to indicate if this is a Paper File or an Electronic File.

Section III: Assessments & Individual Employment Plan/Individual Service Strategy: We have added a place for the Objective Assessment Summary (OAS), Career Exploration and Job Readiness Training
