The following ITA forms are attached below. Click on each form to open and download.
1) Training Exploration Form: This information is needed so the training provider and/or vendor can submit their billing information to International Scholarship & Tuition Services (ISTS).
All clients must explore at least 2 training programs and complete the Training Exploration form for each program, prior to applying for and ITA. A list of ITA training providers & programs is available on Illinois workNet at:

2) UPDATED! Financial Statement: Complete this financial statement and upload it to the ISTS site with the ITA Application. 4/13/26 UPDATES: Added O*NET search, Maximum Tuition and Fees auto population, auto calculation of amount exceeding tuition and fees cap, and more. (See new ITA Financial Statement Question Guidance document attached)
3) Attendance Sheet: Give the attendance sheet to the client to give to the Training Provider for use in the billing process.
